Schedule a call with our intake team for screening and data collection​. We can answer any questions you have about services and insurance,.

Required documents:​

• Consent Package
• Insurance Card
• Photo ID
• Custody/Court Orders (if applicable)

Evaluation Assessments

Once you’ve completed your consent package and we have all the necessary documents, we schedule virtual or in person assessments.

Assessment 1: Diagnostic assessment with a licensed mental health profession (roughly 90 minutes).

Assessment 2: Psychiatric Assessment with a Psychiatric Nurse Practitioner (PNP) (roughly 60 minutes).

Disposition & Assignment

After you’ve had the diagnostic and medication evaluations, our Medical Director and Clinical Managers review your case and discuss diagnosis, enrollment, level of care, recommendations for treatment/services and possible referrals.
